HomeAudits & Investigations
Audits & Investigations
Two post-implementation services, separate from delivery: independent audits to assess ongoing CAS compliance, and investigations to understand incidents, failures and losses.
Compliance doesn’t stop after commissioning.
Continuous monitoring and regular audits help an operation check that controls remain in place and the system performs as intended after commissioning. Fleet Intelligence monitors connected CAS indicators between reviews; our people provide separate, independent on-site audits.

MHSA Regulation 8.10.1
We review collision-avoidance controls and evidence against South Africa’s MHSA Regulation 8.10.1, which addresses preventing pedestrian injuries from collisions with trackless mobile machines. Where significant risk exists, it sets out detection, warning and intervention measures; subregulation 8.10.1.2(b) specifies safe-speed retardation and automatic braking for underground diesel machines.

Our team has experience with the ISO 21815 collision-warning and avoidance series, and can help review system evidence, verification records and operational performance against the applicable scope.
For 2.13.1 oversight
Give your 2.13.1 appointed engineering team a clearer view of CAS status, open findings, corrective actions and verification evidence — for more informed ongoing oversight.
Keeping your 2.13.1 peace of mind. A clear evidence trail helps appointed engineering teams see what has been checked, what needs attention and what has been closed out.
Find what happened. Verify what changes.
When an incident, system failure or recurring loss occurs, we reconstruct the event from available records and site context, identify contributing causes and track corrective actions to evidence-based close-out.
Incidents involving people
Review vehicle-related injury and near-miss events through data, site conditions and the work as performed.
CAS performance
Examine late or missed warnings, interventions that did not act, and issues that affect operator trust.
Machine failures
Trace brake, electrical, engine or drivetrain failures to mechanical, system or operating causes.
Operational losses
Investigate repeated wear, fuel use and equipment damage that affect safe and effective operations.
A loop that actually closes.
Findings only matter once actions are verified. Every investigation ends with a corrective action register — owners, deadlines and the evidence that will show it worked.
Audits and investigations are separate from CAS implementation consulting. They begin after commissioning or when an operational event requires independent review.
